CHIP FORMS From the Dept. of Social Services: "South Dakota Children are eligible for FREE health insurance through the Department of Social Services if they are under age 19 and income eligible for CHIP (Children's Health Insurance Program). CHIP covers doctor appointments, hospital stays, dental and vision services, prescription drugs, mental health, rehabilitation, etc. Children who already have private health insurance may also be eligible for CHIP paying the deductibles, co-payments, and other medical services not covered by the policy."
Forms are available in both offices for those who are interested. If you would like one sent home with your student, please give us a call and we will do that for you.
